About the role

An opportunity exists for a suitably qualified Registered Nurse or Allied health professional to join the Ambulatory Aged care Programs. This position is a fixed term part time position. The role provides case management / care coordination to clients in a community setting on Transition Care, Restorative Care and GEM in the Home programs. These 3 programs form the Ambulatory aged care Programs.

The Clinical Care Coordinator is an integral part of the multidisciplinary team. They will work in consultation with Physiotherapists, OT’s, Team Assistants, Speech pathologists, Dieticians, Geriatricians and Team leaders to implement best practice for AACP clients.

This position will provide the successful applicant with valuable Case Management/Care Coordination skills which is a sought after quality in community health programs.

We are looking for

Current registration with AHPRA in the relevant discipline (if applicable)
Relevant Tertiary Qualifications in Nursing or one of the Allied Health disciplines and eligability for professional body membership
Minimum of three years clinical experience
Excellent time management
High level interpersonal and communication skills and a demonstrated capacity to work successfully in a team environment
Drivers Licence essential

Selection Criteria

Essential : Current Registration with the Australian Health Practitioner Regulation Agency in the relevant discipline (if applicable).
Essential : Relevant Tertiary Qualifications in Nursing or one of the Allied Health disciplines and eligibility for professional body membership
Essential : Minimum of three years clinical experience and the ability to practice flexibility across acute, sub-acute aged care and community sectors.
Essential : Demonstrated ability to case manage clients and coordinate services within a care coordination model.
Essential : The ability to work collaboratively as an individual practitioner within an interdisciplinary team environment including the ability to initiate and maintain effective relationships.
Desirable : Well-developed understanding and experience of allied health and nursing practice delivered in the rural setting, particularly within a regionally integrated service delivery model
Essential : Excellent interpersonal, written and verbal communication skills, including computer literacy
Essential : A satisfactory National Criminal History Record Check, Victorian 'Employee' Working with Children Check and completion of a Commonwealth of Australia Statutory Declaration will be required from prospective candidates prior to commencement of employment.
